The mission of the Behavioral Health Division is to help individuals and families maintain a balanced lifestyle by providing professional and confidential behavioral health services and resources that are culturally appropriate to the urban Native American community and other populations.

After Hours:

If you do not have an emergency but have a medical or behavioral health concern that you believe cannot wait until our regular clinic hours, please call for advice.  Dial your regular clinic number (below), and your call will be forwarded to our answering service.  You will be prompted to select the option for connecting with an after-hours provider.  The answering service will notify the on-call medical provider of your question or concern and you will receive a response from the provider:

NATIVE HEALTH’s Behavioral Health Department is licensed by the Arizona Department of Health Services as an outpatient treatment center and for misdemeanor domestic violence offender treatment services. NATIVE HEALTH accepts AHCCCS Complete Care (ACC) plans.

Intakes

Intakes are required for all patients seeking any Behavioral Health Department service (with the exception of Talking Circle).  Staff are available for walk-in and pre-scheduled intakes (scheduled in two-hour increments), Monday through Friday, 8:00 a.m.- 5:00 p.m.

List of required documents to bring for intake:

  • Patient’s identification (or that of parent/guardian, if patient is a minor)
  • Proof of guardianship for patient (if patient is a minor or if patient is an adult who has a legal guardian assigned)
  • Health Insurance or AHCCCS identification
  • Tribal ID or CIB
  • Please bring three current paycheck stubs if you will be applying for the NATIVE HEALTH Discount Fee Program
